Officials Data Cards
The USTA requires officials to submit their data card every year to retain certification.  On odd years (as is 2007) the bottom of the card must be completed by an eye doctor, attesting the official has 20/20 vision (corrected or uncorrected). 
Last year, the USTA Officials Department in New York began enforcing a strict policy on submitting data cards.  Officials who do not turn in a card will have their certifications revoked.  If it is submitted late, the official’s certifications will be reduced to provisional.  Unfortunately some officials found this out the hard way.  So please be sure the front of your data card is complete – along with the doctor’s signature (if required) and information.  Mail the original data card by October 10th, 2006 to:
